Adult Passport Application (In-person Lodgement Only)

If you are applying for a First Adult Passport, Replacement Passport, Replacement of Emergency Passport, Replacement of Lost or Stolen Passport, or Replacement of Seriously Damaged Passport, you must submit a full application including the following documents in person. Postal lodgement option is not available.
Additional requirements apply for passport applications with name changes.

* Replacement Passports have the same expiry date as the passport being replaced. Adults with more than 2 years validity remaining on their current passport which has full visa pages are eligible to apply.

 

Required documents:

  • Application form (Can also be obtained by contacting your nearest Australian Mission)
    (Print the form single sided using the "fit to page" setting to ensure the whole page prints correctly)

  • Guarantor
    (Australian citizen or foreign citizen currently employed in approved occupation)

  • 2 Australian Passport photos (Endorsed and signed by the Guarantor on the back of 1 photo)

    • Do not attach photos to application form, attaching photos to the application form with glue, paperclips, staples or bulldog clips can mark or damage the photo making them unusable

  • Current or most recently expired Australian Passport (unless lost or stolen)
    (Including Child or Emergency Passport. Ignore if applying for first passport)

  • 2 IDs with photo and/or address
    (Japanese Residence Card / Driver’s Licence / My Number Card / Others)

  • Passport Fees    

                         Fees at the Australian Embassy Tokyo

                         Fees at the Australian Consulate-General Osaka

 

If born in Australia:

 

If born overseas (including in Japan):

  • Foreign Birth Certificate (In Japan: Koseki Tohon / Shusshotodoke Kisaijiko Shomeisho / Others)
    (Original document only, copy not acceptable)

  • Professional English Translation (NAATI) of Foreign Birth Certificate
    (ONLY if the foreign birth certificate is not in English. Original document only, copy not acceptable)

  • Proof of Australian Citizenship (Australian Citizenship Certificate)
    (Original document issued by Department of Home Affairs only, copy not acceptable)
