Child Passport Application (Under 18 years old - not inclusive) or Child Passport Renewal (In-Person Lodgement)

Child Passports cannot be renewed with a streamlined renewal form, a full regular application form is required.

 

Required documents:

  • Application form (Can also be obtained by contacting your nearest Australian Mission)
    (Print the form single sided using the "fit to page" setting to ensure the whole page prints correctly)

  • Guarantor
    (Australian citizen or foreign citizen currently employed in approved occupation)

  • Complete parental consent page by both parents (or original court order for exemption)
    (Signatures must be witnessed by eligible witnesses. Page 4 of application form)

  • 2 Australian Passport photos (Endorsed and signed by the Guarantor on the back of 1 photo)

    • Do not attach photos to application form, attaching photos to the application form with glue, paperclips, staples or bulldog clips can mark or damage the photo making them unusable

  • Current or most recently expired Australian Passport
    (Including Child or Emergency Passport. Ignore if applying for first passport)

  • Parent’s Passport (Australian or Foreign)
    (Original document only, copy not acceptable)

  • Parent’s photo ID with address
    (Japanese Residence Card / Driver’s Licence / My Number Card)

  • Passport Fees    

                         Fees at the Australian Embassy Tokyo

                         Fees at the Australian Consulate-General Osaka

 

If born in Australia:

 

If born overseas (including in Japan):

  • Foreign Birth Certificate (In Japan: Koseki Tohon / Shusshotodoke Kisaijiko Shomeisho / Others)
    (Original document only, copy not acceptable)

  • Professional English Translation (NAATI) of Foreign Birth Certificate
    (ONLY if the foreign birth certificate is not in English. Original document only, copy not acceptable)

  • Proof of Australian Citizenship (Australian Citizenship Certificate)
    (Original document issued by Department of Home Affairs only, copy not acceptable)

 

If you have checked all of the above, please click here to make an appointment at the Australian Embassy Tokyo or at the Australian Consulate-General in Osaka to lodge your application in person.

 

Please note:

  • Child applications aged 15 or under must be lodged by a parent or person with parental responsibility for the child.

  • Child applications aged 16 or 17 must be lodged by the applicant together with a parent or person with parental responsibility for the child.
